Med Gas Braze Continuity Forms

When brazing through your employer, use form #720-21 Rev 02-22-22 (Record of Continuity). Print onto company letterhead and fill out completely. Be sure to include your BRAZE DATE, which must be prior to your expiration date. Submit this form along with your $10 payment to the Education Fund Office.

Weld Continuity Forms

Use this form for updating your Weld Continuity every six months. Fill in the form and have your contractor representative sign, then send it to the Education Fund Office. Your Weld Continuity will be submitted to the United Association on your behalf.
